lang="xx" is the originaltitle,mat8861 just didnt add a language code
this is most likely because the original title could be in one a many different languages.
you can fix this yourself using the rex postprocessor,see the documented configuration files available on the downloads page.
this will change lang="xx" to lang="en",u can change "en" to whatever language code you want.

i had a look at the rex documentation and the titleoriginal isnt a supported element so the lang code will have to be set in the ini itself to something other than "xx"

you should use something like

using {xx} add nothing if the element is empty.
what you have would for example add a .(period) if there was no country data or ,(comma) if no category.
title original is not a valid element.
the original title is added as a secondary title usually with a different language attribute.
example

the info your showing is from the index page which webgrab only gets the title from.

all we can so is separate the title from the rest using things like .[space] or [space[( or :[space]
its not 100% accurate 100% of the time because the level of information changes from show to show and channel to channel.
could we just scrub the title as your showing it...sure.
that would make you happy but tomorrow some other user will want the title a different format.
trying to keep 1 ini working is enough work.
multiple different versions for mutliple users is not going to happen.
if you really want it your specific way there a manual on the downloads page that explains how to create ini's.

if you are the (n) wont be removed for current epg descriptions,only new epg thats added.
only way to update all epg is run webgrab in update f(force) once,then change it back to update i.

i did update files above to fix another problem i found so replace your files.
no revision number change.
you probably need to refresh your data(delete your guide.xml or run a force update on the channel for this ini).
**change the timezone on the site {xx} line to your local timezone.
i changed it to mine for testing and forgot to change it back to Europe/Vilnius
i updated to your tv.telia.lt.ini file i get
